The Community Health Improvement Plan is a long-term, systematic effort to address public health problems and make sustainable improvements in the health and well-being of all people in Wood County. It is based on the results of the Community Health Assessment that was conducted in 2021, a data-driven examination of the health of adults, youth and children who live in Wood County.
The health priorities identified for the CHIP reflect critical steps for improving health in Wood County and were selected through a series of facilitated workshops attended by leaders from various local organizations that impact community health.
At these meetings, attendees will learn about the most important health issues impacting Wood County residents, how CHIP priorities were chosen, and organizations that will be involved in developing plans to drive sustainable improvements in those areas. Attendees will also learn about opportunities to get involved in efforts to make a positive impact on the health of the community.
